Getting Raido buttons to work

I am trying to find a way to use radio buttons to identify what inputbox I am getting data from. I have 6 radio buttons and 6 inputs. I want the user to be able to push a button and based on what radio button is selected get the data from the corresponding input. There are several different buttons but each button will need to collect data from the input based off what radio button is selected.

see the $input1 in the code I want to be able to change this based of what radio box is selected
