Convert one data into another

I Have designed the form With Koda, and now I don't know how to go on.

I want to make the user input a data inside the first text box and make another data depending on what he typed.

for example, I input the string ABCDEFGHIJ, and the program returns:

And make the same but for other video webpages, letting the user choose the server by clicking on a radio placed at the left of the window. And I want to make another form appear when they click a button.
