Download the HTML Help Workshop from Microsoft (where it says download htmlhelp.exe). Build your TOC page and optionally the index page. Compile it. It's that simple. The only part that will take some time to get right is the TOC.Edit: I should have added this part.You already have a good example of how to build the file just by decompiling the existing SQL chm file. It doesn't take any special software to do that. Just open the Run dialog and type hh.exe /? and I'm sure you will figure it out. Think "decompile".
