need mouse button help

i have been trying for a while and seem to be getting no where on this.

i want:

if mouse button 3 is pushed to send alt

and if mouse button 4 is pushed to send Shift

pretty much turning those 2 buttons into shift and alt when the program is running

any help would be nice
