Option Menu/ToolBar script, Can anyone help?

I'm looking for a Script that can be used as an Option Menu/Toolbar in a programmed AutoIt script, That I can use like a tickbox, That chooses what parts of my Main Script get used. I've Googled it, and done extensive Searches on these forums, And I cannot find the script. Please if anyone has a script like this, Could you please help? Example Image bellow. Thank you in advance
